Stage 3 - Care Programme Approach (CPA) discussion
A care plan should then be discussed for the patient and carer based on their needs. This can take place at the hospital and sometimes in the patients home. Not all the people listed may be involved. Some may visit a patient in their home prior to the meeting in order to assess the patients and carer's particular needs. If a CPA has not been discussed, first ask the professionals involved in the patient's care to clarify the situation. If this is not done, contact your local Alzheimer's Disease Society for help.
